Patient safety data includes the following categories:

  • Monitoring Programs/REMS
    • This database contains monitoring programs content as reported by the manufacturers and may include:
      • Brief program description
      • Summary of adverse effects or other risks that prompted a REMS requirement
      • Prescriber requirements
      • Pharmacy requirements
      • Dispensing requirements (patient enrollment)
      • Quantity limits
      • Medication Guide indicator and PDF (according to REMS)
      • Monitoring requirements
      • Documentation
      • Other items not covered in sections above (i.e., some REMS programs require drug wholesalers to register)
  • Black Box Warnings (BBW)
    • When a drug has an associated Black Box Warning, the text of the warning is provided as an XML File that can be formatted to display or print in end-user applications.
  • Medication Guide Indicator
    • When a drug is associated with a Medication Guide (outside of a Monitoring Program/REMS program), the Medication Guide is provided for display or to print in end-user applications.
  • Acetaminophen Ingredient
    • Prescription and over-the-counter drugs that include acetaminophen as an ingredient are identified. This information is useful when developing patient monitoring and/or counseling programs within end-user applications.
  • Pseudoephedrine Ingredient
    • Prescription and over-the-counter drugs that include pseudoephedrine as an ingredient are identified. This information can be used when developing inventory, distribution, patient monitoring, or other programs, as appropriate within end-user applications.
  • Morphine Equivalent Dosing (MED), including milligram morphine equivalent (MME) values
    • Used to help identify and avert potential over-utilization issues and adverse drug events. MED normalizes the dose and route of each opioid a patient is taking against morphine, using a standard conversion table, to determine if a patient has exceeded a set dosing threshold.
  • Tall Man Drug Names
    • Drug names for which Tall Man Lettering is defined by the Institute for Safe Medication Practices (ISMP) and the US Food and Drug Administration (FDA) are identified. This information can be used when developing end-user applications, such as implementation of additional safety checks to help ensure the clinician, pharmacist, or nurse is prescribing, dispensing, or administering the intended drug. Tall Man Drug Names are provided in several Medi-Span drug databases and supplemental files.
